Admission Requirements

Must be at least 18 years of age.

Copy of High School Diploma, or GED.

Must be able to read, write, and speak English. All classes are held in English.

US citizen or legal resident of the US.

Hepatitis B antibody blood test result.

PPD Test result.

Malpractice insurance must be purchased through Phlebotomy Plus.

(2) Passport Photo.

Registration Fee.

All must be presented to the admissions office on the first day of class.
